It is when he does not want to take a nap or trying to get the subject off of him getting in trouble he will hurt himself, say ouch and start to cry, he is three.

You are the mother, am i right? you gotta tell him he can't do that, teach him to be responsible. if he continues, give him medical care (duh) but don't be overly sympathetic unless he doesn't do it purposefully.
you gotta teach him that, if isn't naughty, he doesn't have to hurt himself. and if he does, he gotta face up to it, or he'll be in more trouble. (worked with me)
